This policy recognizes that laboratories may contain biological, chemical, and/or radiological hazards, as well as mechanical, electrical, and thermal hazards from laboratory equipment. Laboratory animals may pose additional risks, including bites, scratches, and exposure to allergens or zoonotic agents. These hazards pose risks not only to those performing laboratory work, but also those who are present within the laboratory while work is on-going.

The University of Wisconsin-Madison prioritizes safety and strives to provide a safe learning environment for everyone. Children come into contact with the UW through various programs, camps, and events. On December 19, 2011, the Governor signed Executive Order #54 which requires that all University of Wisconsin System employees report incident of child abuse and neglect. In addition, this policy extends that obligation to certain volunteers and contractors performing services for the UW.

This policy prohibits the acts of sexual harassment and sexual violence (including sexual assault, dating violence, domestic violence, stalking, and sexual exploitation) in all programs and activities of the University. Individuals who engage in such acts, hereafter referred to collectively as sexual harassment and sexual violence, are in violation of this policy and are subject to disciplinary action. This policy also prohibits retaliation against individuals who report sexual harassment or sexual violence, who assist others in reporting, or who participate in university proceedings related to such a report. Individuals who engage in retaliation are subject to disciplinary action.

This policy defines four types of information incidents: (1) loss or theft, (2) intrusion by malware or unauthorized access via the network, (3) physical intrusion, and (4) all others. Each type has criteria that make a possible incident “reportable.” The policy requires the reporting of reportable incidents unless one of the exceptions applies. The Incident Reporting and Response Procedures are the implementation of the policy.

Cybersecurity is a collective responsibility that requires policy that applies to all components of UW–Madison. Threat, vulnerability, and the likelihood of exploitation are complex and unique to specific business processes and technologies. Cybersecurity risk is measurable depending on quantified or classified aspects of the data; characteristics of the information system; the definitions and characteristics of internal or external threat, system, or environmental vulnerabilities; and the likelihood that the event or situation may manifest itself within a given application, information system or architecture.

Program information includes details about participants and staff, as well as other information, that should be viewed only on a need-to-know basis. The safest place to store this information is in a Box or Google drive with limited access (that requires a NetID login).

Do not store sensitive information on portable devices or in department networks.

You must save program information, including but not limited to:

  • Participant and staff rosters
  • Participant enrollment and registration records
  • Staff training records
  • All incident reports
  • The program registry/attendance records (for residential programs)
  • The program health log (for residential programs)
